SPORTS PHYSICALS
CURRENT 6th graders:
If you are planning to participate in Cross Country, Wrestling, or Soccer in 7th grade your 7th grade school physical must be completed by a physician by the first day of school.
CURRENT 7th graders:
If you are planning to participate in any sport at Sunrise Middle School for 8th grade, a sports physical completed by a physician is required. The blue physical form is available at Sunrise Middle School and at the clinics. Return the blue Sports Physical form to the Sunrise Middle School office after it is completed.
CURRENT 8th graders:
If you are planning to participate in any sport at Kearney High School for 9th grade, a sports physical completed by a physician is required. The blue physical form is available at Sunrise Middle School and at the clinics. Return the blue Sports Physical form to Kearney High School after it is completed.
Appointments may be scheduled any time after MAY 1.
