ATTENTION 2014-2015 7th graders

ALL students entering the 7th grade are required, by law, to have a school physical and Tdap shot before Aug 14. 2014, the first day of the 2014.2015 school year.  Do not wait until summer to schedule as you may not be able to get it done before the first day of school.  Students cannot start school without proof of having the Tdap shot and will be excluded.

The physical form was sent home with students on March 6th. 2014.
Please return the form to the school office only after the physical has been completed.

STUDENT ACTIVITY TICKETS  Activity Tickets admit students to all regular season football, basketball, volleyball games and wrestling meets at Sunrise and Horizon Middle Schools and Kearney High School.  Activity Tickets can be bought at the school offices.

LEAVING SMS DURING THE SCHOOL DAY

If your child needs to leave school for an appointment or other pre-planned absence, please send a note along with your child. The note should be brought to the office so we can give your child a pass to leave class at that time. This will eliminate calling into the classroom and interrupting class when the time comes for them to leave. When you arrive to pick them up, please come into the office and sign your child out. Upon returning, your child must go to the office and sign back in and receive a pass to get back into class. If your child will be leaving with someone other than yourself, please state on the note who will be picking them up.

MEDICATION REMINDER
ALL over-the-counter and prescription medication taken at school must have the school form completed by a parent/guardian before the medication can be given.  Medication must be in the original container. Notes from parents will not be accepted in place of the school form.

Sunris
e is collecting BOX TOPS, APPLE MARKET RECEIPTS, & FAMILY FRESH (Sunmart) RECEIPTS.  Please send your box tops and grocery receipts to school with your student. With your help the school can purchase additional items for the classrooms.
