7th school physical
ATTENTION 2015-2016 7th graders
State law requires proof of a 7th grade physical and immunizations (Tdap) for ALL students entering 7th grade before Aug 14. 2015, the first day of the 2015 school year.á Do not wait until summer to schedule an appointment as you may not be able to get it done before the first day of school.á Students cannot start school without proof of having the Tdap shot and will be excluded. The completed physical form can be brought Sunrise Middle School during the summer or during registration on August 4th and 5thOnly return the health form after the physical has been completed.
If you do not have health insurance or Medicaid for your child, contact the newly opened HelpCare Clinic (phone 308-224-2392), the 1st week in June to see if your child qualifies for a free physical and immunizations. School physicals will be conducted on June 24, July 8 and July 10 from 9a.m. to noon. Immunizations will also be provided on these dates through Mid NE Community Action Partnership, suggested price for immunizations is $18.á

Hastings College Scholars Academy June 22-27, 2015
A six-day residential camp for current Gifted & Talented 7th & 8th grade students. They experience small academic classes that challenge their true potential.áClasses offered are Art, Language/Performing Arts, Mathematics, Science, and Robotics. Small group activities include improvisations, swing dancing, board games, volleyball, laser tag and more. Two of the large group activities are Mini-Olympics and a trip to the Hastings Water Park.á Brochures and registrations are available in the counseling office at Sunrise or online at www.hastings.edu/hcsa. Application deadline is May 4, 2015.

8th grade SPORTS PHYSICALS
available after May 1, 2015.

Sports physicals are required to participate in 8th grade athletics during the 2015-2016 school year. Nebraska Schools Activities Association (NSAA) Bylaws mandate that schools can accept a students' physical dated afteráMay 1st for the following school year.
A physical card is available at the clinics or can be picked up from the Sunrise office.á Return completed physical cards to the Sunrise Middle School office.á
The Help Care Clinic will provide sports physicals for students that qualify on July 13, 15, 17th from 9:00 to noon.á This is available only to students with no insurance or Medicaid.á Please call to see if your student qualifies, 308-224-2392

SUNRISE INFORMATION
CELL PHONE POLICY Cell phones can not be used by students in the school building. Student cell phones must be turned off and in the student's locker during the school day. 1st offense: phone is confiscated and a parent must pick it up. 2nd & additional offenses: a parent may pick up after 5 school days from the school office.

STUDENT ACTIVITY TICKETS

Activity Tickets admit students to all regular season football, basketball, volleyball games and wrestling meets at Sunrise and Horizon Middle Schools and Kearney High School.á Activity Tickets can be bought at the school offices.á

LEAVING SMS DURING THE SCHOOL DAYá If your child is leaving school for an appointment or other pre-planned absence, please send a note with your child. All students are signed out at the office. Students must stop at the office and sign back in when they return. If your child will be leaving with someone other than yourself, please indicate who will be picking them up in your note.
MEDICATION REMINDERáá ALL over-the-counter and prescription medication taken at school must have the school form completed by a parent/guardian before the medication can be given.á Medication must be in theáoriginal container. Notes from parents will not be accepted in place of the school form.

Sunrise is collecting BOX TOPS, APPLE MARKET RECEIPTS, & FAMILY FRESH (Sunmart) RECEIPTS.á Please send your box tops and grocery receipts to school with your student. With your help the school can purchase additional items for the classrooms.

Lost and found items will be placed on a table in front of the office the last week of every month.á The items left will be donated to the needy.
