7th school physical
ATTENTION 2016-2017 7th graders
Students cannot start school without proof of having the Tdap shot and will be excluded.
ALL students entering 7th grade are required, by law, to provide proof of a 7th grade physical and a Tdap booster before Aug 24. 2016, the first day of the 2016 school year.á The completed physical form can be brought to Sunrise Middle School anytime after completing or during information days on August 15th and 16thOnly return the health form after the physical has been completed.

Tuesday, May 10 @ 7:00: band information night about instrument rental and auditions for students interested in playing percussion.

SUNRISE INFORMATION

CELL PHONE POLICY Cell phones can not be used by students in the school building. Student cell phones must be turned off and in the student's locker during the school day. 1st offense: phone is confiscated and a parent must pick it up. 2nd & additional offenses: a parent may pick up after 5 school days from the school office.

Lost and found items will be placed on a table in front of the office the last week of every month.á The items left will be donated to the need.

STUDENT ACTIVITY TICKETS

Activity Tickets admit students to all regular season football, basketball, volleyball games and wrestling meets at Sunrise and Horizon Middle Schools and Kearney High School.á Activity Tickets can be bought at the school offices.á

LEAVING SMS DURING THE SCHOOL DAYá If your child is leaving school for an appointment or other pre-planned absence, please send a note with your child. All students are signed out at the office. Students must stop at the office and sign back in when they return. If your child will be leaving with someone other than yourself, please indicate who will be picking them up in your note.
MEDICATION REMINDERáá ALL over-the-counter and prescription medication taken at school must have the school form completed by a parent/guardian before the medication can be given.á Medication must be in theáoriginal container. Notes from parents will not be accepted in place of the school form.

Sunrise is collecting BOX TOPS, APPLE MARKET RECEIPTS, & FAMILY FRESH (Sunmart) RECEIPTS.á Please send your box tops and grocery receipts to school with your student. With your help the school can purchase additional items for the classrooms.

Sunrise students will receive a school memory book free of charge! The books are bought through school fundraising efforts and Student Council. The memory books are handed out the last day of school.

8th grade SPORTS PHYSICALS
available after May 1, 2016.

Sports physicals are required to participate in 8th grade athletics during the 2016-2017 school year. Nebraska Schools Activities Association (NSAA) Bylaws mandate that schools can accept a students' physical dated afteráMay 1st for the following school year. The clinics have the forms. Return completed physical cards to the Sunrise Middle School office.á

The Help Care Clinic will provide sports physicals for students that qualify. This is available only to students with no insurance or Medicaid.á Please call to see if your student qualifies, 308-224-2392
